Welcome to Eastern Arizona College!

Steps to Complete Admissions

1. Complete and Submit the Admissions form Online

- Pay particular attention to Proposition 300 information.

Once You Have Received Your Student ID

1. Complete the WUE form

2. Provide Proof of Residency (if you are a resident of one of the following states: AK, CA, CO, HI, ID, MT, ND, NM, NV, OR, SD, TX, UT, WA, WY).

3. Complete the New Mexico Waiver and Provide Proof of Residency (if you are a resident of New Mexico).

Housing

1. Housing Application-Log in to eac.erezlife.com with your MyEAC credentials to complete.

Academic Requirements

1. Have an Official Copy of Your Final High School Transcripts Sent to the Admissions & Registration Office at Eastern Arizona College.

This must be sent directly from your high school and must be signed. The date of your high school graduation must be indicated.

2. Have an Official Copy of College Transcripts Sent to the Admissions & Registration Office if you have taken ANY college courses.

This must be sent directly from the college.

3. If transcripts indicate placement testing is necessary, you will want to schedule an appointment with the Testing Center.

Insurance and Medical Requirements

1. Schedule a Physical Exam with your doctor.

2. Print and Have Your Doctor Complete the Physical form Provided by EAC.

3. Print and Complete the Insurance information form (requires parent or guardian signature if under the age of 18*).

4. Include a Copy (Front and Back) of Your Insurance Card.

Athletic Forms

1. Print, complete, and sign Athletic forms.

2. Print, read, and sign Athletic Commitment to Excellence form indicating that you understand and agree to abide by this policy.

3. Print, read, and sign Substance Abuse Education and Testing Program policy

-Initial and sign the Consent to Drug Testing and Authorization for Release of Test Results, indicating that you understand and agree to abide by this policy.
